A Concord woman is accused of creating a fake letter from an insurance agency that resulted in her receiving more money from an area hospital than authorities say she was entitled to.
Tanisha Lozada, 25, has been charged with one count of obtaining property by false pretense. Officials said additional warrants were also served on Lozada.
The arrest was announced by the state Department of Insurance, and stems from treatment Lozada received last June after a car accident.
In a news release, the insurance department said Farm Bureau Insurance overpaid Carolinas HealthCare System by $2,010.73 for Lozada's care. The balance was owed to Lozada, the release stated.
Investigators allege Lozada created a document on fake letterhead from the Farm Bureau and faxed it to the hospital system, instructing it to send the refund directly to her. According to the insurance officials, Farm Bureau did not know of the letter from Lozada and also issued her a check for the $2,010 that was owed.
The Department of Insurance said it has 20 law enforcement officers that look into possible cases of insurance or bail bonding fraud. Since 2009, the agency said it has made 575 arrests that have resulted in 320 criminal convictions so far. More than $47.9 million has been collected in restitution and recoveries, the department said.
