A restaurant inside SouthPark mall is at the bottom among this week's health inspection reports.
The inspection report for Maki of Japan in the food court shows food debris on utensils that were supposed to be clean, and no hand soap anywhere in the store.
Inspectors also noted that eggs, chicken and beef were all stored at the wrong temperature.
Maki of Japan scored an 82.
The other low score this week was at Fairview Grill on Park Road.
Inspectors found that a fridge didn't work, meaning that cheese and chili stored inside weren't being kept cool. Inspectors also found flies in the kitchen and a dish machine that needed repairs.
Fairview scored an 82.5.
